Behavioral Intelligence for Predictive Care

01 · The problem

The fall is rarely the first sign.

The slower walk. The disrupted night. The hesitation in a familiar routine. The quieter voice. Alone, each means little. Together, and against that person’s own baseline, they are measurable drift, and it can begin months before the fall. You see the resident at the visit, the screen, the score. The drift happens in between, where no one is measuring.

02 · The voice · Speech, available now

The voice changes before the chart does.

Speech is the first domain of the Healthy Habits Index™, and the first available today. A short voice assessment establishes an individual baseline. Repeated assessments identify meaningful change against it. Index re-checks whether the change persists, adds resident context, and prioritizes who may warrant attention first. No hardware.

Forty-five seconds of speech doesn’t give your team another dashboard. It gives them a shorter list.

1. Assess

Dorothy Williams, 84, a fictional resident, takes a 45-second guided session by phone or tablet. Three in week one set her baseline; she repeats at Day 60. No clinician is needed to administer it.

Dorothy's baseline change, persisting

2. Learn

Dorothy's baseline, not an average. Index compares Day 60 with her own baseline and re-samples 48 to 72 hours later before anything is escalated.

100 assessed → 12 need a closer look

3. Prioritize

Who, why, how confident, and a suggested first step with an owner. First steps are drawn from the peer-reviewed literature on the pattern observed and applied under the community's clinical protocol; the care team owns the decision.

Why Index surfaced her. Several speech-derived indicators associated with cognitive change moved together at Day 60 relative to her own baseline, and the change reproduced on the 48-to-72-hour re-sample.

Why this first step. The change followed a new sedative. Medication effects are among the most common reversible causes of cognitive change in older adults, so the review comes before the screen.

Index does not diagnose. This finding identifies a resident who may warrant professional evaluation; the care team decides.

Day 60 · 100 residents

Designed to support clinical judgment, not replace it. The Healthy Habits Index™ identifies meaningful change against an individual’s baseline so care teams know where to look. It does not diagnose, and suggested first steps are applied under each community’s own clinical protocol. Canary Speech supplies a speech-analysis engine used within Speech; the baseline, confirmation protocol, priority logic, first steps, and report are Index.

Each figure is one expected case of mild cognitive impairment. The orange ones are the ones primary care diagnoses. Liu et al., 2024.

Why the voice first

8 in 100

expected cases of mild cognitive impairment are diagnosed in primary care. Roughly 92 of 100 go undiagnosed.

Your blind spot is the resident who looks fine.

Detection waits for a visit, a clinician, and a reason to look. Index does not replace your screen. It tells you who may need one.

03 · The night · Sense™, Domain 02: Sleep · In development · Target pilot launch Q4 2027

The night knows first.

Sense™ is being developed to add Sleep, the second domain of the Healthy Habits Index™, through passive overnight sensing. A bedside lamp uses millimeter-wave radar and acoustic features to observe sleep fragmentation, breathing pattern, bed exits, and time to rise. No camera. Nothing worn.

Dorothy's breathing pattern shifts while she sleeps.
Chest-wall movement pauses, then resumes.

3:14 AM

A third trip to the bathroom. Her baseline is one.

6:05 AM

Getting out of bed takes forty seconds longer than last month.

6:06 AM

The walk to the bathroom is slower, and has been for six days. That is the day it surfaces.

What the wellness director sees: her name near the top of the list, sleep flagged, overnight movement shown as context, six days persisting. What happens next is the clinician's call.

Illustrative scenario for a capability in development. Sense observes movement signals overnight; Mobility as a commercial domain of the Index remains in development.

04 · The pattern · Speech + Sleep

One change is a moment. Two are a direction.

A change in speech alone may warrant a closer look. Add what happened overnight, for the same person, against her own baseline, and the care team has context instead of a single data point. Cross-domain context is what Index is being built to provide. It surfaces patterns for professional review; it does not diagnose.

Domains 03 to 06 · In development

And these are only the first two domains.

Mobility, hygiene, hydration, and routine are in development. As each is added, the Index is designed to bring all six together into a single longitudinal view of functional change, one measurable domain at a time.

Healthy Habits Index™

In development · one longitudinal behavioral biomarker of functional health

05 · The Index

Normal isn't a number. It's a name.

As additional domains validate, Index is designed to bring them together into one longitudinal view of functional change, against the same person’s own baseline. Slower gait says something changed, not what; six domains together add the context a single signal cannot.

Drift Atlas™

A chart shows where she is. Drift shows which way she's going.

Drift Atlas™ is the population-scale map of how function changes over time, built as permissioned longitudinal evidence accumulates. It is the layer that turns a longitudinal behavioral biomarker into something healthcare has never had for functional decline: a reference for how patterns of functional change unfold over time.

06 · The decision · Who it's for

Where the drift begins, and why it pays to see it

Life plan communities and senior living

When a fall leads to a move-out, the clinical cost becomes an occupancy cost too. A short list instead of a hunt is nurse time returned to the floor. Start in independent and assisted living, where the drift begins.

PACE and plans at risk

When you carry the cost of the hospitalization, an earlier signal on the frailest participants is the value. Index fits the interdisciplinary team's existing review.

Home health agencies

You see the patient at the visit. Index is being built to make meaningful change between visits visible, starting with speech and expanding to passive Sense™ monitoring.

    Why Index

You are not buying sensors. You are buying a decision.

Every signal Index captures exists to answer one question for a care team: where does attention belong this week.

    Individual, not average

Every resident becomes her own baseline. Change is measured against the person, not a population norm.

    Between visits, not at them

Index is built around what changes between traditional assessments. Speech establishes and re-checks the baseline today; Sense™ is being developed to extend that visibility passively.

    Low friction first. Passive next.

Speech starts the relationship today with a 45-second session and no hardware. Sense™ is being developed to extend it passively: millimeter-wave radar, no camera, no video, nothing worn.

    08 · The last chapter

    For John.

Lt Col John Waddell was a decorated Army combat veteran. In 2016, in Suffolk, Virginia, he began to decline, and none of us had a word for what we were watching. He walked a little slower. His nights broke up. He drank less than he should have and kept his routines a little less. Nothing that looked like an emergency. Everything that turned out to be one.

He would not wear the pendant. It was stigmatizing, and it only worked after he had already fallen. When the falls came, he moved to assisted living. Within six months of losing his independence, we lost him.
The signs had been there for months, to anyone with a way to see them. That sentence is the whole company. Build the way to see them.
